Driver #2: Ramona Sansalone 78 of Minotola NJ

Driver #3: Theresa Dodd 23 of Millville NJ

Driver 1 stated she was north on Delsea Drive approaching Elmer Road traffic signal.  She stated Vehicle 2 slowed and began to stop abruptly in front of her.  She stated she attempted to slow/stop however crashed into the rear of Vehicle # 2.  Driver 1 was issued citation 0614-VC-8086 for Careless Driving, 39:4-97, with a court date before the Vineland Municipal Court. 

Driver 2 stated she was slowing/stopping for the traffic in front of her at the traffic light, Elmer Road, when Vehicle 1 crashed into the rear of her vehicle. 

Driver 3 stated she was slowing/stopping for the traffic in front of her at the traffic light, Elmer Road, when Vehicle 2 crashed into the rear of her vehicle.

Driver #1 was attempting to enter the roadway (South Delsea Drive) from a private drive, Team Nissan, 1715 South Delsea Drive.  She stated there was a very large motor home (vehicle X) parked on the east side of the roadway (shoulder), just south of the driveway, blocking her view.  She stated she inched out of the driveway to attempt to look for traffic when vehicle #2 collided with her vehicle.  Driver #1 claimed to have no injuries, however complained of pain to her forehead.  Vineland EMS was on scene and driver #1 refused to be treated.   

 

Driver #2 stated he was traveling north on South Delsea Drive, when vehicle #1 pulled out of the driveway.  He stated he attempted to avoid colliding with vehicle #1 by swerving into the oncoming lane.  He stated it was clear of traffic, but was not able to avoid colliding with vehicle #1.  Driver #2 claimed to have no injuries, Vehicle #2 passenger, Jennifer Ford, advised she had complaint of pain to her right side of face and right side of body, possibly from the airbags.  Ford advised she wanted to get her one-year-old daughter treated as she was unsure if she was injured.  Vineland EMS was on scene and they were transported to Inspira Health Network - Vineland.   

 

Vehicle #1 had extensive damage to the front passenger side and was towed by South side Auto Body.  Vehicle #2 had extensive damage to the front drivers’ side and was towed by Giancola.

Driver #2: William Catlett 51 of Millville NJ

Driver #1 stated she was traveling north bound on South Delsea Drive when the traffic was backed up due to a collision up the street (north bound). Driver #1 stated she turned around to go south bound due to the traffic being stopped. Driver #1 stated when she turned around, Vehicle #2 which was traveling south bound attempted to “go around” her vehicle (#2) and side swiped it.

Driver #2 stated he was traveling south bound on South Delsea Drive approaching West Elmer Road when the north bound lane on South Delsea Drive was backed up due to a collision up the street (north bound). Driver #2 indicated as he was traveling south bound a vehicle (#1) in the north bound lane did a U-turn in front of him and entered his lane of travel. Driver #2 indicated at that time Vehicle #1 struck his vehicle (#2) at a right angle. Vehicle #1’s front passenger side bumper struck Vehicle #2’s driver side door.

Both drivers and the occupant had no complaint of pain.

Vehicle #1’s sustained moderate damage to front passenger side bumper.

Vehicle #2’s moderate damage to the driver side door.

 

Driver #1 was issued summons # 0614-VC-009893 for 39:4-125 (Improper U-turn).

 

NOTE:     I recorded through my MVR the roadway approaching the crash scene in my low profile Vineland Police Traffic Unit patrol vehicle, Tango 4, equipped with a MVR. All statements were recorded utilizing the same.

Driver #1: Debra Vargo 56 of the 1200 block of Roosevelt Blvd.

Driver #2: Vanessa Mendez 44 of Landisville NJ

Driver #1 stated she was stopped at the stop sign on South Spring Road at the intersection with South Lincoln Avenue.  Driver #1 explained she looked both ways prior to making a left turn and felt that she had enough of a clearing to make her turn.  Driver #1 indicated when she pulled into the roadway she collided with Vehicle #2 who was in the South bound lane on South Lincoln Avenue. 

Driver #1 complained of head pain, Vineland EMS responded and assessed Driver #1 however she refused treatment and transport. Driver #1 was transported to Vineland Inspira by her husband after he arrived on scene. 

Vehicle #1 sustained front end damage and was towed from the scene by Giancola Towing. 

Driver #1 was issued summons #0614-VC-009731 for 39:4-144 (Stop Street) 

 

Driver #2 stated she was traveling south on South Lincoln Avenue when Vehicle #1 who was on South Spring Road pulled out in front of her causing the collision. 

Driver #2 complained of left hand pain and rib pain, passenger complaint of right had pain, Vineland EMS responded, assessed Driver #2 and passenger however they refused treatment and transport. 

Vehicle #2 sustained front end damage and was towed from the scene by South Side Towing.

Witness stated he was traveling south on South Lincoln Avenue directly behind Vehicle #2. Witness explained he observed Vehicle #1 pull out of South Spring Road onto South Lincoln Avenue and in front of Vehicle #2 causing the collision.

Location: 20 W. Park Ave.

Driver: Jaclyn Phillips 65 of the 4000 block of Robin Road

Driver# 1 stated that she was pulling out of a parking stall and looked away from the front. She said when she turned her head to look forward she noticed she was heading for a pole. Driver#1 said she applied her brake but struck the pole before stopping. 

Investigation at the scene revealed that vehicle#1 was moved from point of impact prior to my arrival. Vehicle#1 was parked in a parking stall just east of the light pole. Driver#1 pulled out of the parking stall in a south west direction and struck the light pole about 20 feet west of the where she was parked. Vehicle#1 sustained damage to the front end. The metal light pole was not struck. The car hit the cement base at the bottom of the light pole. 

The cement base of the light pole was not damaged. Driver#1 said she was not injured from the accident. Collision reportable due to the amount of damage to the front end of vehicle#1.

Driver #1: Joseph Moore 77 of Temple Road

Driver #2: Brynn Hebert 33 of Newfield NJ

Driver #1 stated that he was stopped at the intersection of W Plum St and N 3rd St (Stop Sign) facing East bound. Driver #1 stated that he looked both ways then accelerated into traffic in an effort to proceed East on Plum St. During this time, Driver #1 stated that he struck Driver # 2's vehicle which was traveling North on 3rd St. He indicated that he did not notice Driver #2's vehicle until the last moment and he was unable to avoid colliding with her vehicle. Driver #1 refused medical treatment at the scene. His vehicle sustained minor damage to the middle passenger side.

Driver #2 stated she was traveling North on 3rd St towards the intersection of Plum St. As she crossed the intersection of Plum St, Driver #2 stated that Driver #1 entered the intersection causing her to collide with the passenger side of his vehicle. Driver #2 claimed that Driver #1 never came to a complete stop before crossing the intersection. Driver #2's vehicle sustained minor damage to the front bumper.

As a result of this collision, I was able to determine that Driver #1 was at fault for failing to yield to Driver #2 who did not have a stop sign.  Driver #1 was issued a summons for failing to Yield to Right of Way (0614-VC-005369).

No Tows were needed at the scene.

Driver #1: Brian Baker 43 of Bridgeton NJ

Driver #2: Grace Rapp 72 of Port Elizabeth NJ

Driver #1, operated by Brian Baker, stated the following.  Brian stated he was traveling east bound on W Landis Avenue and was crossing over Rt. 55.    As Brian proceeded over Rt. 55, he observed vehicle #2, operated by Grace Rapp, coming off the off-ramp of Rt. 55 traveling east bound on W Landis Ave.  Brian stated he continued to travel east bound, when he observed Grace's vehicle begin to merge into his lane.  Brian stated he activated his horn several times to warn Grace, but she continued into his lane.  Brian quickly pushed his brakes, and checked his mirrors to attempt to make a lane change, but before he could change lanes, Grace's vehicle had entered his lane, causing him to strike her vehicle.  After striking her vehicle, Brian stated her vehicle then turned towards the Ramada Inn and went over the curb striking a light post.

Driver #2, operated by Grace, stated the following.  Grace stated she was traveling north on Rt. 55, an entered the off-ramp to travel east on W Landis Ave.  As Grace exited the off-ramp, she stated traffic was clear for her to enter the roadway.  As Grace entered the roadway, she stated her vehicle was suddenly struck from behind, sending her vehicle in the direction of the Ramada Inn.  Grace related as she traveled towards the Ramada Inn, her vehicle jumped the curb and caused her to strike a lamp post approximately 15ft behind the curb.

Terri Seeds (856-696-3800) who is a representative from the Ramada Inn arrived on location to make note of the damage.

Driver #1's passenger, Brianna Baker, complained of head pain and was transported to INSPIRA Medical Center by EMS to be treated for her injuries.  Brian had no complaint of pain at this time.   His vehicle also sustained moderate damage to the front end.

Driver #2 and her passenger, Elaine Morton-Rankin, had no complaints at this time.  Her vehicle sustained moderated damage to the rear, front, and undercarriage.

No summonses were issued at this time.

Driver #1: James Corlett 43 of Erma NJ

On February 21, 2016, at approximately 1:35PM I was dispatched to 3849 South Delsea Drive, Vineland, BJ’s Wholesale Club, parking lot for a motor vehicle collision.  Upon my arrival I met with the owner of vehicle #2, Anna Slowinski, who advised when she returned to her parked vehicle, an unknown female subject approached her.  She stated she told her that a vehicle struck her vehicle and then left the scene.  The unknown female subject provided New Jersey registration DL 8725.   

 

I observed minor scuff marks to the rear drivers’ side fender, of the unoccupied vehicle.  At this time I was told by BJ’s Wholesale Club they did not have video surveillance of the incident.   

 

I then contacted Egg Harbor Township Police Department regarding the registered owner.  An Officer responded to the residence and I later received a phone call from Officer Fessler of the Lower Township Police Department.  Officer Fessler advised James Corlett responded regarding the above investigation.  Officer Fessler observed very minor damage to the front drivers’ side corner bumper of his vehicle, NJ DL8725.  Officer Fessler advised the driver (Corlett) did not feel or notice the collision.  He responded to Lower Township Police Department after being contacted by his brother in Egg Harbor Township.  Corlett claimed to have no injuries, along with his passenger.   No summons was issued.

Driver #2: Adan Pinedo 22 of Landisville NJ

 

Driver#1 stated that she was traveling south in the left lane at the Landis Ave exit of Walmart (1070 W. Landis Ave.). Driver#1 said she was making the left turn to go east on Landis Ave. when suddenly vehicle#2 came through. Driver#1 said she struck vehicle#2 on the left side.  She said the light was green for her while she was exiting Walmart.

Driver#2 stated that he was traveling east on Landis Ave in the right lane approaching the traffic signal in front of Walmart. He said he did not notice the red light and went through the intersection in front of Walmart. Driver#2 said he was struck by vehicle#1 while going through the intersection.

Investigation at the scene revealed that both vehicles had moved from point of impact prior to my arrival. Area of impact was determined by the statement of both drivers. Area of impact was approximately 23 feet north of the south curb line of W. Landis Ave and approximately 400 feet west of the west curb line of Moyer St.

Both drivers stated they and their passengers were not injured. Vehicle#1 sustained damage to the right front. Vehicle#2 sustained damage to the left rear. 

Driver#1 was written a subpoena. Driver#2 was issued a summons (VC009192) Failure to Observe Red Light.

Vehicle # 1 was traveling south on the property of 20 West Park Avenue.  Driver # 1 stated she did not see vehicle # 2 and when she was turning onto the property she hit vehicle # 2/

Driver # 2 was traveling north on 20 West Park Avenue. Driver # 2 stated vehicle # 1 crashed into his vehicle.

The front right passenger of vehicle # 2 had a nose bleed and pain to her left eye. Vineland EMS responded on scene and transported her to IHN. No other injuries were reported.

Vehicle # 1 HAD FRONT END DAMAGE to the bumper and vehicle # 2 had damage to the left side of the vehicle. Vehicle # 2 was left on scene and vehicle # 1 was driven from the scene.

Driver # 1 was issued a motor vehicle summons for Careless driving 39:4-96, summons # VC-0007106.

Vehicle # 2 insurance company is Gainsco Auto insurance; 3333 Lee Parkway, suite 1200 Dallas TX

Driver #1: Yvette Rodriguez 50 of the 300 block of S. 8th Street

 

Driver #1 stated she was heading eastbound on E Almond St towards SE Blvd. Driver #1 stated a vehicle was approaching her from the opposite direction so she moved to the right side of the street as far as she could (The street has vehicles parked along both sides). As she moved to the right, she struck a vehicle parked along the south curb line of E Almond St.

 

Vehicle #2 was parked and unoccupied on the south curb line of E Almond St facing west. Vehicle #2 was struck by Vehicle #1.

 

 

  No injuries reported by the occupant of Vehicle #1 at this time. No occupants in Vehicle #2.

 

 

Vehicle #1 sustained moderate damage to the passenger’s side front bumper and disabling damage to the right front tire. Vehicle #2 sustained moderate damage to the passenger’s side front bumper and tire.

 

 

Summons #0614-VC-008352 (Careless Driving, 39:4-97) issued to Driver #1.

Driver #1: Dawn Byers 43 of the 1000 block of Tammie Terrace

 

Driver #1, said she was traveling eastbound on Elmer Road, when a deer ran in front of her vehicle. She advised she was unable to avoid hitting the deer due to it coming out of nowhere.   

Driver #1, sustained damage to the front portion of her vehicle, but was still able to driver her vehicle away from the scene.

No injuries were reported at the scene. Also no summons were issued.

Driver #1: Jacquelyn Steiner 59 of Williamstown NJ

Driver #2: Christopher Dorsey 26 of Hammonton NJ

Driver # 1, stated she was facing east on West Sherman Avenue and in a complete stop waiting in traffic when she was rear ended by vehicle # 2.  

Driver # 2, stated he was traveling eastbound on West Sherman Avenue when he struck vehicle # 1, which was in a complete stop.  
 
Both drivers stated they did not suffer any injuries during the crash.


Vehicle # 1 had damage on the rear bumper and Vehicle # 2 had damage on the front. Vehicles # 1 and vehicle # 2 were driven away from the scene by their drivers.  


Point of Impact (P.O.I) was determined by the statement of both drivers. P.O.I was approximately 100 feet east of the west curb line of Route 55 and 15 feet north of the south curb line of West Sherman Avenue.


Driver # 1 was issued with traffic summons # 0614-VC-007174, Careless Driving (39:4-97)

Driver #2: Isaid Jimenez 30 of W. Chestnut Ave.

Driver# 1 stated that she was stopped in traffic south bound on S. Delsea Dr. when suddenly she was struck in the rear by vehicle#2.

Driver#2 stated that he was traveling south on Delsea Dr. and was slowing down for traffic stopped in front of him. Driver#2 said he was attempting to stop but his brakes failed. He said he was unable to stop and struck vehicle#1 in the rear. Driver#1 said that was the first time he had brake problems and did not recently have work done on his SUV.

Investigation at the scene revealed that both vehicles were in the original point of rest after impact. Area of impact was determined the statement of both drivers. Area of impact was approximately 200 feet north of the north curb line of and approximately 10 feet east of the west curb line of S. Delsea Dr. Vehicle#2 was drivable but was towed because driver#2 said his brakes failed.

Driver# 2 stated he was not injured from the accident. Driver#1 was transported to Inspira for back pain.

Driver#1 was written a subpoena. Driver#2 was issued a summons (VC009194) for Careless Driving and a summons (VC009195) for Unlicensed Driver.

Location: S. State Street and Landis Ave.

Driver #1; Nelson Rosa 52 of the 1000 block of E. Park Ave.

Driver #2: Maryann Steinborn 62 of the 1000 block of E. Park Ave.

On February 22nd, 2016 at approximately at 1:00pm I was dispatched to the area at S State Street and E Landis Avenue for a two vehicle collision with injuries, Vineland EMS was also dispatched to the scene. Upon my arrival Vineland EMS was actively treating Driver #1. Prior to him leaving in the ambulance I spoke with Driver #1 who was extremely alert. He informed me was stopped at the stop sign on S State Street beginning to slightly turn right so that he can continue to travel south on S State Street. As he entered the intersection he stopped briefly due to oncoming traffic at which time vehicle #1 hit him from behind, causing the collision. 

Driver #1 had a complaint of neck and back pain. He was transported to Inspira Medical Center for further evaluation.

I then spoke with Driver #2 who informed me she was traveling south bound on State Street beginning to make a slight right hand turn to continue driving south on S State Street. Driver #1 assumed Driver #2 was going to make a complete right hand turn on to E Landis Avenue and continue to drive on E Landis Avenue. When Driver #2 entered the intersection he stopped abruptly and Driver #2 hit him from the rear, causing the collision.

Driver #2 did not have any complaint of pain.

Vehicle #1 sustained moderate damage to the rear passenger side bumper consisting of a dent approximately 6 inches in diameter.

Vehicle #2 sustained minor damage to the front passenger side bumper consisting of scratches and scuff marks, the front license plate was bent.

Vehicle #1 was driven away from the scene by Driver #1's spouse who had arrived after the collision. Vehicle #2 was driven away from the scene by Driver #2.


Driver #2 was cited for careless driving Summons vc-9196

Driver #1: Jose Reynoso 18 of the 1900 block of N. East Ave.

Driver #2: Rolando Serrano 72 of the 300 block of W. Peach Street

Driver #1 stated he was traveling southbound on N East Ave towards W Oak Rd. He stated he observed vehicle #2 stopped on the shoulder of the road on the east side facing north bound. Once he approached Ithaca St, Vehicle #2 attempted to make a U-turn in the roadway, Driver #1 attempted to apply his brakes but struck vehicle #2 on the passenger side quarter panel. Vehicle #1 had moderate damage to the front end. Driver #1 had complaint of pain to his back and was transported to IHN by Vineland EMS. Passenger #1 had complaint of pain to his left foot but refused medical attention by Vineland EMS. Vehicle #1 was towed by All-Tow.

Driver #2 stated he was stopped on the shoulder of the road on the east side facing north bound. Driver #1 observed Vehicle #1 coming southbound but he believed he had enough time to make a U-turn. Once he attempted to make a U-turn, he was struck by Vehicle #2. Vehicle #2 suffered minor damage to the passenger side quarter panel. Driver #2 and Passenger #2 claimed no injuries.

Driver #2 was issued a summons for 0614 V 122237 for Improper Turn (39:4-123).

Driver #1: Guillermo Gutierrez-Vazquez 38 of the 300 block of W. Almond Street

Driver #2: Hit and Run

Dispatch advised that a subject named Lorenzo Vazquez called and stated he was driving vehicle 2, vehicle 1 struck him from behind in the area of N Delsea Dr and W Oak Rd. Driver 2 advised he was following vehicle 1 and last observed it at W Elmer St and S Second St. Driver 2 then disconnected.

Ofc. Maslanich located vehicle 1 pulling into 329 W Almond St and observed driver 1 exiting the vehicle. Sgt Flores and I responded on scene, a short time later.

Driver 1 stated he is having a problem with an unknown Mexican male and this Mexican male is driver 2. As he was driving north on N Delsea Dr approximately 20 feet south of W Park Ave, he observed driver 2 who was directly in front of him. Driver 2 then out of nowhere, slammed on his brakes coming to an immediate stop, so that driver 1 could strike him from behind. Driver 1 attempted to stop, but was unable to. Driver 1 lightly tapped vehicle 2 from behind. Driver 1 did not have a phone and was attempting to get home to call this department. No visible damage was noted to vehicle 1.

Dispatch contacted driver 2 to see where he is. Driver 2 stated he is not in the area and will not respond to this department to speak with me, disconnecting the phone.

Vehicle 2 was described as a 4 door Kia, silver in color.

Driver 2 was described as a Mexican male, unknown any other description.
